Our focus is on providing care in the places - and for the people - that are often the hardest to reach with strong community partnerships and a well-trained and supported caregiver workforce.

Home and Community-Based Care

Since 1985, Homebridge has provided over 15 million hours of in-home care services to thousands of In-Home Supportive Services (IHSS) beneficiaries in San Francisco with complex health, behavioral and social needs. These services are available in permanent housing, shelters or wherever a person lives.

In-Home Supportive Services (IHSS) Contract Mode

The Homebridge model of IHSS care is made possible through a contract with the City and County of San Francisco to provide IHSS services to an eligible population who cannot coordinate their own care.

Homebridge IHSS care is delivered through a team model that trains and supports skilled caregivers, their supervisors, and wrap-around support staff. This coordinated approach ensures stability, sustainability, and quality care, especially for individuals at significant risk of adverse health and wellness issues due to medical and behavioral complexity.

Expanding Access to IHSS

San Francisco’s partnership with Homebridge and its tiered continuum of support for individuals has led to innovative service delivery options that increase access to IHSS care. In partnership with the City and County of San Francisco, Homebridge provides IHSS services to residents living in Permanent Supportive Housing (PSH) sites through the Collaborative Caregiver Support Team (CCST) program and other initiatives that aim to prevent a recurrence of homelessness and improve health outcomes. Homebridge links residents to IHSS enrollment, IHSS services and connections to vital home care support.

Enhanced Care Management

In January 2024, Homebridge added CalAIM’s Enhanced Care Management (ECM) to its services, a Medi-Cal benefit that provides comprehensive case management for high-risk individuals and families. ECM allows Homebridge to provide more robust, integrated health and home care to our IHSS clients.

Caregiver Development & Support

We believe that investing in the caregiving workforce is the foundation for improved care and quality of life for everyone in our community. We aim to reduce barriers to caregiver employment through high-quality training and skills development and by advancing caregiver career ladder opportunities into community-based care and certified healthcare.

Caregiver Training

Homebridge is California’s largest provider of caregiver training. We offer a broad array of education and advancement opportunities for caregivers, primarily through the California IHSS Career Pathways program and CalGROWS initiative. Based on our decades-long expertise in providing skilled, compassionate, client-centered care and using real-life examples, we teach day-to-day skills in four pathways: general health and safety, cognitive impairments and behavioral health, complex physical care needs, and transitions to in-home and community-based care.

Caregiver Community Connection

Homebridge Community Connection is a newly launched custom online platform for caregivers who often work alone or provide care to loved ones, friends and family members at home. The platform offers caregivers a space to connect, seek guidance and discover a wealth of shared experiences and support.
