The expansion strengthens access to community-based care for Alameda County residents and aligns with CalAIM initiatives to improve whole-person health outcomes.

Alameda, CAHomebridge announced the expansion of its direct care services into Alameda County through a new partnership with Alameda Alliance for Health, marking the organization’s first direct care expansion outside of San Francisco.

Through this partnership, Homebridge has been approved as a Community Health Worker (CHW) provider in Alameda County under CalAIM. The designation enables Homebridge to deliver community-based, non-clinical support services for eligible Medi-Cal members, including care coordination and assistance addressing social needs that affect health outcomes.

Services will be delivered at select housing sites across Alameda County beginning this month.

This expansion supports CalAIM’s goal of better integrating health care and social services to improve outcomes for individuals with complex health, behavioral health, and social needs. It also reflects ongoing efforts by Alameda Alliance for Health to expand access to community-based care and strengthen local service capacity for eligible Medi-Cal members.

“When people have safe, stable housing and trusted community-based support, their health outcomes improve,” said Alameda Alliance CEO Matt Woodruff.  “By embedding CHW services within housing settings, this partnership brings care, stability, and access directly into the communities we serve—where health, healing, and home meet.”

Homebridge Senior Director of Growth & Innovation Bazil Fonseca said the partnership represents a key step in the organization’s growth.

This expansion is part of Homebridge’s broader strategy to build regional capacity for community-based care across California and support integrated care models that address both health and social needs.
